Admissions Criteria

In general the following criteria must be met:

You must have a primary psychiatric disorder

You cannot be a convicted violent felon

You cannot be a pedophile, have a history of fire setting or a history of serious violence toward others

You must have a current physical examination completed within 30 days prior to the date of admission

You can have a secondary diagnosis of a substance abuse disorder

You must complete an application for services and send SLS information about your prior treatment history

Admission Process

If you would like to be considered for admission you need to do the following things:

Contact our Intake Department to have your questions answered and to inquire about availability. We generally have more applicants than we have available openings so planning ahead is important.

Complete our application and send it to SLS by fax, email or postal mail.

Our Intake Department will contact you to set up a phone interview with a member of our clinical staff.

You may schedule an onsite visit to meet with our staff.

Once we have your application, interviewed you, and confirmed funding of your care and treatment we will decide whether you are an appropriate admission to our programs. We will tell you of our decision by phone.

Upon acceptance you will have three days to decide whether or not you want to be admitted to SLS.

If you decide to be admitted we will schedule your admission as soon as possible. If you are covered by insurance your admission may be subject to pre-certification by your insurance company.

On the day of your admission you will come to our main offices and meet with the Director of Care Management who coordinates all admissions. You will then meet with residence staff and clinical staff.

Fees

Fees are based on which program you are applying to and can be obtained by calling our Intake Department.

Insurance

SLS Residential participates in many major insurance and managed care company networks, including Cigna Behavioral Health, MHN, Oxford, United Behavioral Health and Value Options. We also accept out of network insurances, which cover only a fixed percentage of the total charges. Our staff will help you determine if you have any benefits for SLS Residential and if you do, we will seek authorization from your insurance company to use your benefits.
